David C. Dahlquist Scholarship Application Form

Requirements for Application:

  1. Must have a High School GPA of 3.0 or higher. ( Submit copy of your transcript. )
  2. Applied to and accepted for admission to a College/University. ( Submit copy of acceptance notification. )
  3. Have taken at least one Choir class taught at Payson High School.
  4. One Letter of Recommendation from a non school, non family source which is a character reference. This should focus on the applicant as a person, not a student or musician.
  5. Using your Choir experience as a resource, attach an essay explaining how music and service go hand in hand. ( Essay is not to exceed two single side pages, double spaced, 12 point font. )
